SCHEDULE

PRIMARY HEALTHCARE FUND
OUTPATIENT CARE SERVICES
Scope Access Point Tariff Access Rules
1. Consultation, diagnosis, and treatmentLevel 2, 3 and select Level 4 primary healthcare referral facilities as contracted by the Authority to offer primary healthcare services.KES. 900 per person per annum1. All registered beneficiaries will be mapped to a Primary Care Network (PCN) at the point of registration, where they can access services in any facility within their Primary Care Network. The global budget capitation shall be allocated based on the population III the Primary Care Network (PCN). Distribution of the Funds shall be done at the end of the month based on patient visits, weighted by disease treated.
2. Prescribed laboratory investigations
3. Basic radiological examinations including X-rays, and ultrasounds.
4. Prescription processing, drug administration and dispensing.
5. Health education and wellness, counselling, and ongoing support as needed.
6. Management of acute and chronic conditions for endemic/ local cases
7. Management of non-communicable diseases (NCDs), enteric infections; neglected tropical diseases (NTDs), sexually transmitted infections (STI's) and HIV/AIDS
8. Mental Health education, counselling and psychosocial support services.
9. Screening for common health conditions.
10. Minor outpatient procedures in the procedure room or under local anaesthesia.
11. Immunization as per the Kenya Expanded Program on Immunization (KEPI) schedule and recommended special vaccines.
12. Reproductive, Maternal (including antenatal and postnatal care services) and Child Health Services as defined by the Ministry of Health guidelines.
SCREENING & MANAGEMENT OF PRE-CANCEROUS LESIONS
Scope Access Point Tariff Access Rules
Screening services shall include—Level 2-6 with capacity to offer the service1. Human Papillomavirus (HPV) – KES. 3,6001. Screening for common health conditions will be financed under outpatient services.

OPTICAL HEALTH SERVICES

1. Eye health education and counselling.2. Eye health education, consultation and eye tests will be covered under the outpatient services.2. Treatment for refractive errors is limited to beneficiaries below eighteen(18) years in year one of implementation, and the scope will be increased based on utilization and financing indicators.
2. Eye tests include fundoscopy, visual acuity testing, and visual field analysis.
3. Treatment of refractive errors through eyeglasses.

4. Bedside services including physiotherapy, occupational therapy, oxygen supply, medical consumables, and therapeutic nutritional support.   
5. Cross-matching and administration of blood and blood products; derivatives and components, artificial blood products, and biological serum.
6. Post-discharge medication or follow-up within the treatment plan.

MATERNITY, NEONATAL AND CHILD HEALTH SERVICES

1. Aftercare for the mother together with the newborn. 2. Caesarean Section and essential newborn care –KES. 30,0002. Management of complications for the mother and the newborn will be covered within the other benefit packages as needed.
2. Midwifery, including episiotomy care and nursing care. 3. Anti-D Serum – KES. 6,000 
3. Operating, recovery, maternity ward and other treatment room charges including meals and special diets.   
4. Immunization for the newborn.   
5. Intra-admission and post-discharge medication.   
6. Diagnostic laboratory tests.   
7. Administration of blood and blood products; derivatives and components, artificial blood products, and biological serum.   
8. Medical supplies and equipment, including oxygen.   
9. Postnatal family planning.   
10. Management of postpartum infections and haemorrhage, birth traumas and conditions related to childbirth.   
11. Management of neonatal complications child, sepsis, jaundice, prematurity, necrotising, and enterocolitis, among others.   
12. Anti-D Serum for Rhesus-negative mothers.   
13. All other services as defined by the Ministry of Health (MOH) guidelines   
RENAL CARE SERVICES PACKAGE
 
The management of kidney failure due to chronic disease or acute injuries/diseases through;Level 3-6 with capacity to offer the service1. Hemodialysis & Hemodiafiltration KES. 10,650 per session.1. 2 sessions per week for haemodialysis and hemodiafiltration.
1. Dialysis package: 2. Peritoneal dialysis – KES. 85,200 per month.2. 12 monthly sessions for peritoneal dialysis. SHIF will cover expenses up to the specified tariff.
(a) Consultation and specialist's review. 3. Pre-transplant recipient evaluation at an annual limit of KES. 150,000 per household. 
(b) Cost of the temporary catheter and insertion/removal. 4. Recipient surgery – KES. 700,000. 
(c) Nursing care and dialysis services. 5. Donor nephrectomy – KES. 168,000. 
(d) Routine laboratory investigations (Urea Electrolytes and Creatinine (UECs), Full Hemogram, and blood glucose) 6. Post-renal replacement therapy at an annual limit of KES. 200,000 per household. 
(e) Dispensation of medication and maintenance drug including Erythropoietin injection.   
(f) Continuous intermittent dialysis and other dialysis protocols.

MENTAL WELLNESS BENEFIT PACKAGE

The mental health services cover caters for—Level 2-6Outpatient services will be reimbursed within the outpatient services package at Level 2, 3 and select Level 4 primary healthcare referral facilities as empaneled and contracted by the Authority to offer primary healthcare services.1. Diagnosis and management of behavioral disorders while admitted will be provided for within the inpatient annual limit of 180 days.
1. Mental Health education, counselling and psychosocial support services. Inpatient Services2. Social Health Insurance Fund (SHIF) will cover expenses for Drug & Substance Abuse up to the specified tariff.
2. Screening, management and referral for behavioral disorders, neuro-developmental disorders, and affective and psychoactive disorders. 1. Level 4 – KES.3,360 
3. Rehabilitation for substance-related addictive disorders. 2. Level 5 – KES.3,920 
4. Neurodevelopmental disorders include attention deficit/ hyperactivity disorder (ADHD), autism, learning disabilities, intellectual disability (also known as mental retardation), conduct disorders, cerebral palsy. 3. Level 6 – KES.4,480 
5. Neurodegenerative disorders include Parkinson’s disease, Alzheimer’s disease, Huntington’s disease, amyotrophic lateral sclerosis (ALS), multiple sclerosis (MS), dementia and motor neuron disease among others. Rehabilitation for Drug& Substance Abuse

OVERSEAS TREATMENT PACKAGE

SURGICAL SERVICES PACKAGE
The Surgical Package covers the following surgical and non-surgical procedures-Level 3-6 with capacity to offer the serviceSchedule of tariffs annexed.1. 3 minor procedures per household per year.
1. Pre-operative admission and care.  2. 2 major procedures per household per year.
2. Minor, major and specialized surgical procedures including organ transplants and implants.  3. 1 specialized procedure per household per year.
3. Administration of blood and blood products; derivatives and components, artificial blood products, and biological serum.   
4. Management of intraoperative complications and complications following the surgical procedure.   
5. All additional medical or surgical service required during the postoperative period because of complications that do not require additional trips to the operating room.   
6. Postoperative visits - follow-up visits during the postoperative period that are related to recovery.   
7. Post-procedure pain management.   
8. Post-operative ward physiotherapy.   
9. Supplies - except for those identified as exclusions.   
10. Miscellaneous services - items such as dressing changes; local incisional care; removal of the operative pack; removal of cutaneous sutures and staples, lines, wires, tubes, drains, casts, and splints; insertion, irrigation and removal of urinary catheters, routine peripheral intravenous lines, nasogastric and rectal tubes; and changes and removal of tracheostomy tubes.   
11. Critical care services related to the surgical procedure but within the same case definition.   
12. Diagnostic tests and procedure, including diagnostic radiological procedures related to the surgery.   
13. Immunosuppressive therapy for organ transplants.   
MEDICAL IMAGING AND OTHER INVESTIGATIONS PACKAGE

HEMATOLOGY & ONCOLOGY SERVICES PACKAGE

1. Treatment through Chemotherapy, Radion therapy, Hormonal therapy, and Immunotherapy.   
2. Consumables, stoma appliances, premeds, and post meds, Chemotherapy pump   
3. Routine and specialized laboratory investigations (Total Blood Count (TBC), Urea Electrolytes Creatinine (UECs) and Liver Function Tests (LFTs)).   
4. Therapeutic apheresis (Aphaeretic platelets, red cell exchange)   
5. Screening, cross-matching, and administration of blood and blood products; derivatives and components, artificial blood products, and biological serum   
6. Radiological services as necessary as part of oncological treatment planning and staging.   
7. Management through surgical interventions.   
8. Management of precancerous lesions.   
ORAL HEALTH SERVICES

AMBULANCE EVACUATION SERVICES

1. Cardiac and pulmonary arrest  2. Care for up to a maximum of 24 hours after which the Social Health Insurance Fund (SHIF) benefits are activated for paid-up members.
2. Major trauma (severe burns, head injuries, severe wounds, multiple fractures)   
3. Shock states (Haemorrhagic, septic, dehydration)   
4. Altered level of consciousness (unconsciousness, confusion) Severe respiratory distress   
5. Seizures.   
6. Chest pain (Acute Coronary Syndrome/Pulmonary embolism) Acute Cerebrovascular Accidents (Strokes).

CRITICAL ILLNESS PACKAGE

PALLIATIVE CARE SERVICES

This benefit aims at enhancing the quality of life for patients with serious chronic or life threatening illnesses which includes symptom management, psycho-social support, caregiver support and end-of-life care.Level 3-6 with palliative care units1.   2. Level 5 – KES. 3,9202. SHIF will cover expenses up to the specified tariff.

ASSISTIVE DEVICES

Provision of assistive devices to support patients with permanent physicaland/or sensory disabilities and progressive chronic conditions such as Parkinson’s Disease and multiple sclerosis.Level 4-61. Hearing aids – KES. 55,0001. One device per household per year, except for therapeutic footwear, which has a limit of two per household per year.
  2. Wheelchair – KES. 15,0002. Hearing aids are limited to individuals aged 18 years and below and will only be issued once per lifetime.

CHRONIC ILLNESSES

Longitudinal management of chronic conditions and diseases beyond the SHIF limit for—Level 4-61. Medical admission cases will be reimbursed as prescribed under the Medical Inpatient Package1. Must be a paid-up Social Health Insurance Fund (SHIF) member.
1. Medical admission cases. 2. Haemo-dialysis, peritoneal dialysis, pre-transplant evaluation and post-renal therapy will be reimbursed as prescribed under the Renal Care services package in Social Health Insurance Fund (SHIF).2. Shall be covered comprehensively subject to budgetary allocation.
2. Haemo-dialysis and peritoneal dialysis 3. Mental Health Disorders will be reimbursed at the same inpatient benefit package rates above. 
3. Pre-transplant evaluation.   
4. Immunosuppressive therapy post-renal transplant.   
5. Management of cancers.   
6. Inpatient management of mental neurodevelopmental, neurodegenerative, affective, psychoactive disorders and substance addictive disorders and other mental health conditions.   
VERTICALLY FINANCED PROGRAMS
 
Anti-retroviral (ARVs), anti-malarials, anti-Tuberculosis (TBs), and associated tests, family planning commodities, and Kenya Expanded Program on Immunization (KEPI) vaccines will be provided at public facilities and faith-based & private facilities that report to the health information system.
LIST OF EXCLUSIONS: GENERAL AND SPECIFIC
1. Cosmetic procedures or such other procedures that the medical advisor deems cosmetic and any complications arising out of this unless specified in the surgical schedule (Annex 3).
2. Massage (except where certified as a necessary part of treatment following an accident or illness);
3. Treatment by chiropractors, acupuncturists, and herbalists,
4. Assisted fertility treatment by In Vitro Fertilization (IVF) or any other procedure.
5. Vaccines other than those of the Kenya Expanded Program on Immunization (KEPI).
6. Products outside the Kenya Essential Medicine List (EML) and the Kenya Essential Diagnostics List (EDL).
7. Any investigation, injury, disease or illness not specified in the benefits package for the particular Kenya Essential Package for Health (KEPH) level of service.
8. Weight management treatment drugs.
9. Nutritional supplements.
10. Shampoos (except where certified as a necessary part of treatment).
11. Medical costs related to or incurred in a research environment and clinical trials.
12. Travel expenses other than ambulance costs as defined under emergency care and referral services.
13. Stop-smoking aids.
14. Epidemics and pandemics.
15. Hormone replacement therapy (HRT), unless in connection with, and immediately after a pre-authorized surgical procedure or unless otherwise provided for under the terms and conditions of the treatment plan.
16. Admission in a private ward, stay at a sanatorium or old age home, treatment in a spa, sauna, health farm, or similar establishment, stays and/or maintenance or treatment received in nature cure clinics or similar establishment or, convalescent and or rest homes or cures attached to such establishments.
17. Professional fees for medical personnel over and above the prescribed rates.
18. Expenses related to diagnostic equipment including but not limited to such as glucometer or glucose machine.
19. Any other procedure or intervention not listed in the scope of the benefit package.
20. Services that are not directly related to medical treatment such as personal comfort items or luxury services.
